| 内容記述 | 背景・目的：クローン病は原因不明で永続的な治療が必要で、患者には主体的なセルフケア実践が求められるため、看護師はセルフケアを高める支援を行う必要がある。本研究は、クローン病患者へのセルフケア支援内容とその効果を文献から明らかにし、患者のセルフケアを高めるために有効な支援について検討することを目的とした。 研究方法：医学中央雑誌web、PubMed、CINAHL を用いた文献検討を行い、本テーマに即した16 文献について検討した。 結果：クローン病患者へのセルフケア支援内容は、セルフモニタリングの強化4 件、食事栄養療法3 件、ストレスマネジメント3 件、運動療法3 件、生物学的製剤に対するアドヒアランス向上2 件、禁煙1 件であった。ストレスマネジメントに関する支援の効果として、症状や疾患活動性の改善、QOL の向上がみられた。運動療法に関する支援の効果として、骨密度や筋力、身体活動機能の向上、ストレスの低減、疾患活動性の低下、 QOL の向上がみられた。 結論：クローン病患者のセルフケアを高めるには、双方向性による継続的な支援に加え、知識や情報の提示に留まらず具体的な実践スキルを提供することが有効であることが示唆された。 |

| 内容記述 | Background and purpose: Crohn’s disease requires permanent treatment due to an unknown cause, and patients are required to practice self-care proactively, so nurses need to support patients in improving self-care. This study aimed to clarify the content and effects of self-care support for Crohn’s disease patients from literature and to examine effective support for improving patients’ self-care. Research method: A literature review was conducted using the Central Medical Journal web, PubMed, and CINAHL, and 16 articles related to this topic were examined. Results: The self-care support provided to Crohn’s disease patients included strengthening self-monitoring in four cases, dietary nutrition therapy in three cases, stress management in three cases, exercise therapy in three cases, improving adherence to biological agents in two cases, and smoking cessation in one case. The effects of support for stress management were improvements in symptoms and disease activity, and improved QOL. The effects of support for exercise therapy were improvements in bone density, muscle strength, and physical activity function, reduction in stress, decreased disease activity, and improved QOL. Conclusion: It was suggested that in order to improve the self-care of patients with Crohn's disease, it is effective to provide not only knowledge and information, but also continuous, interactive support and specific practical skills. |
