| 内容記述 | 本研究の目的は、フットケア介入の終了直後および6 ヶ月後の足部の変化を明らかにし、フットケアの継続の必要性を検討することである。研究対象は6 週間のフットケア介入が終了し、かつ6 ヵ月後に追跡調査できた高齢者6 人とし、フットケア介入終了直後と6 ヵ月後に基本的属性、足部の形態・機能、立位・歩行機能を調査した。そして、2 つの時期における足部の形態・機能や立位・歩行機能を対象内比較およびWilcoxon の符号付順位検定により比較した。その結果、循環機能に関連する足部の変調や足底部の角質化は再び出現していた。また、ケアによって改善していた足底部の触圧覚はほとんどの測定部位で閾値の上昇がみられた。循環機能のうち、末梢血流量は両足ともに有意な血流減少がみられた。また、足底部の皮膚表面温度は左側が有意に低下し、右側は低下する傾向がみられた。立位バランス機能の指標である開眼片足立ちの保持時間は有意に短縮し、FRT のStart-End Point も有意に短縮した。歩行機能の指標である10m 最大速歩行は有意に遅くなり、TUG では有意差はなかったものの実行速度が遅くなっていた。さらに、下肢筋力を反映する足趾間把持力は、左右とも有意に低下していた。以上の結果から、フットケアで一旦改善された足部も6 ヶ月間のケアの中断により可逆的に変化することが明らかとなり、短いスパンあるいは定期的、継続的なケアの必要性が示唆された。 | |||||||||||||||||||||||||||||||||||||

| 内容記述 | The purpose of this study was to confirm the condition of feet after six months of foot care interventionhad ended and to consider the need for continuing foot care. The subjects were six senior citizens who hadreceived foot care for six weeks. We investigated the structure and function of feet, standing balance, andwalking performance. We compared the condition of each subject's foot at two levels, and compared it withWilcoxon's matched pair test.As a result of this 6-month pause, a slump in the foot's condition related to circulation andkeratinization of the plantar was observed once again. Moreover, the sensation of touch and pressure of theplantar decreased. In addition, the quantity of peripheral-blood flow decreased significantly to both feet.Surface temperature of the skin fell significantly. Furthermore, two tests that served as an index of standingbalance showed a significant decrease in functional status while two tests that were the index of walkingperformance also showed a decrease in functional status. Moreover, the grip force of toes decreasedsignificantly in both feet.From these results, it is understood that the foot returns to the state prior to foot care when no foot care hasbeen administered for at least 6 months.
