| 内容記述 | 本研究の目的は、精神障がい者の摂食嚥下機能支援に対し、摂食・嚥下障害看護認定看護師（以下、認定 看護師と略す）が自らの経験をどのように認識し、その後にどのような行動をしているのかを明らかにする ことである。5 名の認定看護師を対象に半構造化面接を行い、実践した印象的な支援に対する認識、認識後の行動に焦点を当て、コード化しサブカテゴリー、カテゴリーへと抽象化した。調査期間は2012 年～ 2014 年で、研究協力者は全員女性で、平均年齢は43.2 歳（SD=9.6）、看護師の経験年数21.8 年（SD=10.0）、精神科臨床経験年数15 年（SD=3.0）、認定看護師として精神障がい者の摂食・嚥下障害看護は全員が経験していた。 結果では、支援に対する認識には30 のコード、8 のサブカテゴリーと３のカテゴリー、認識後の行動には11 のコード、2 のサブカテゴリー、1 つのカテゴリーで構成された。支援に対する認識は、精神症状を踏まえた摂食嚥下機能支援を評価する、精神障がい者の口腔ケアは看護師同士の理解や共有が重要である、相手への違和感を機に自身の支援を内省する、であった。認識後の行動は実現可能な支援を継続するであり、ジレンマの中で支援を続行し、自分や相手（医師、看護師、患者）の考えを大切にしていた。 認定看護師は、摂食・嚥下障害看護と精神看護の専門的知識や判断を元に、その支援の評価や、連携の重要さ、感情を手掛かりに振り返りを認識していた。そして、アサーティブコミュニケーションを用いて支援を継続していた。 |

| 内容記述 | This study aimed to identify how certified nurses (CNs) in dysphagia nursing recognize their own experiences and conduct themselves accordingly with regard to swallowing function support for people with mental disabilities. We conducted semi-structured interviews with five CNs focusing on recognition for impressive support they had implemented and their behaviors after the recognition. The interviews were encoded and abstracted, creating categories and subcategories. The study period was from 2012 to 2014. All of the participants were female, aged 43.2 years (SD = 9.6) on average with 21.8 years (SD = 10.0) of nursing experience and 15 years (SD = 3.0) of psychiatric clinical experience. Each of them had had experiences of caring for the mentally disabled with dysphagia as a CN. Recognition for their support was composed of 30 codes, 8 subcategories, and 3 categories, whereas behavior after recognition had 11 codes, 2 subcategories, and 1 category. The results indicated that awareness of support was evident from the evaluation of the support they provided and the patients’ mental conditions, importance of collaboration and information sharing among nurses regarding oral care for such patients, and review of their support quality when they perceived a feeling of discomfort through interactions with patients. As for behaviors after recognition, the CNs provided as much sustained support as they could and continued to support their patients in a dilemma while respecting the opinions of others (doctors, nurses, and patients). The CNs recognized the importance of reflecting on the evaluation of the support they had given, importance of collaboration, and their emotions. They reviewed their experiences based on their judgment and expertise in psychiatry and caring for people with dysphagia. They continued to support their patients using assertive communication. |
