| 内容記述 | 研究目的は、介護老人福祉施設入所者の終末期ケアを、老人病院で死亡した入所者のカルテ調査により明らかにすることである。対象者は介護老人福祉施設入所者33名である。対象者の48.5%は3週間以内、75%が1ヶ月以内に死亡していた。死因は呼吸器系疾患(肺炎)が最も多く42.4%であり、66.6%が1000-1500mlの輸液を受けており、輸液の量と吸引の有無とでは相関もみられた(p<0.001)。看護ケアでは入院前には66.6%の人が経口摂取であり、褥創や創傷もなく皮膚の状態も良好に保たれていた。しかし苦痛に対しての記録は少なく鎮痛剤が投与されていたのは9.1%のみであった。このことは自分から苦痛を訴えることができない対象者の苦痛に対してケアスタッフの関心がうすい結果とも思われた。また対象者の最後の場面に立ち会った家族は45.5%であった。今後施設入所者特に痴呆高齢者が、苦痛を緩和し安心して見守られていると感じることができる終末期緩和ケアを模索していくことが必要である。 | |||||||||||||||||||||||||||||||||||

| 内容記述 | Despite growing consensus that palliative end-of-life care should be provided in the nursing home setting to residents with Alzheimer's and other dementias, many nursing home residents with dementia in Japan continue to be transferred to hospitals during the terminal phase of their illness. The present study investigated all nursing home transfers of patients with dementia who were discharged as dead at a 180 bed geriatric hospital in a major metropolitan area in southern Japan over a one year period (n=33). Ages ranged from 69-100 years (mean 88.2 years). 78.8% of transfers were women. As this hospital 75.8% of transfers died within 30 days of admission. The main causes of death were pneumonia (42.4%) and heart failure (21.2%). 66.7% of transfers were listed as independent in feeding ability by the nursing home. The amount of IV fluid administered was significantly related to whether or not patient received suctioning. Although 48.5% of residents had a nursing note reporting pain only 9.1% received pain medication. 72% of patients received cardiac monitoring and 66.6% received O_2 therapy. 84.8% received antibiotics. A family member was present at 45.5% of the deaths. The findings highlight the growing need for palliative care services. | |||||||||||||||||||||||||||||||||||
