{"created":"2023-06-19T13:30:41.698763+00:00","id":631,"links":{},"metadata":{"_buckets":{"deposit":"5fad9526-8a9e-4a56-b1a6-e7278a657aa8"},"_deposit":{"created_by":14,"id":"631","owners":[14],"pid":{"revision_id":0,"type":"depid","value":"631"},"status":"published"},"_oai":{"id":"oai:jrckicn.repo.nii.ac.jp:00000631","sets":["17:40"]},"author_link":["1289"],"item_3_alternative_title_1":{"attribute_name":"タイトルのヨミ","attribute_value_mlt":[{"subitem_alternative_title":"オウダン ケンキュウ"}]},"item_3_creator_2":{"attribute_name":"作成者","attribute_type":"creator","attribute_value_mlt":[{"creatorAffiliations":[{"affiliationNameIdentifiers":[{"affiliationNameIdentifier":""}],"affiliationNames":[{"affiliationName":""}]}],"creatorNames":[{"creatorName":"MORIYAMA, Masaki","creatorNameLang":"en"},{"creatorName":"守山, 正樹","creatorNameLang":"ja"},{"creatorName":"モリヤマ, マサキ","creatorNameLang":"ja-Kana"}],"familyNames":[{},{},{}],"givenNames":[{},{},{}],"nameIdentifiers":[{},{}]}]},"item_3_date_6":{"attribute_name":"出版年月日","attribute_value_mlt":[{"subitem_date_issued_datetime":"2016-11-01","subitem_date_issued_type":"Issued"}]},"item_3_description_13":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"video/mp4","subitem_description_type":"Other"},{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_3_description_5":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"みなさんこんにちわ。今回は横断研究（横断的研究）についてお話します。\n\n１　横断的研究の概要\n\n横断的研究とは、要因曝露の有無と、疾病（健康障害）の有無を同時に調査し、その関連を明らかにする研究方法です。\nスナップ写真を撮るように、ある一時点で、現状を把握する調査です。\nこの講義室で今アンケート調査を行うなら、それも横断的研究といえます。アンケート調査なら皆さんにも出来るでしょう。調査票の設計に当たっては、まず対象者の性別や年齢・職業などの個人情報を把握した上で、さらに「仮説的な原因・要因曝露」や「仮説的な結果・疾病の有無」についての調査項目（質問）を用意することが大切です。\n\n\n２　アンケートによる横断的研究の演習\n\n以下、この講義室で調査票（アンケート）を配布し、対象者（今日の出席者）100人に調査票に回答してもらえた場合を考えてみます。各調査項目に対し、100人中の何人が、どう答えたかを知ることは、調査票分析の最初の一方です。\n\n1) アンケートの作成・回答・集計\n\n以下、10項目のアンケート調査を考えてみます。各項目の当てはまる方に〇をつけてください。\n①性別　　　　　　　　・女性　・男性\n②喫煙　　　　　　　　・なし　・あり\n③飲酒　　　　　　　　・なし　・あり\n④生活不規則　　　・なし　・あり\n⑤運動不足　　　　　・なし　・あり\n⑥バイト過多　　　　・なし　　・あり\n----\n⑦虫歯　　　　　　・なし　・あり\n⑧腰痛　　　　　　・なし　・あり\n⑨便秘　　　　　　・なし　・あり\n⑩頭痛　　　　　　　・なし　・あり\n\n\n上記のアンケート調査では、項目②から⑥が生活に関連した（仮説的な）要因曝露、項目⑦から⑩が疾病（健康障害）の有無です。調査票を回収したら、各項目について、・なし／・ありの人数を単純集計します。\n\n2) クロス集計と仮説の確認\n\n単純集計が終わったら、二つの項目を組み合わせてクロス集計を行います。どの二つの項目を組み合わせてクロス集計を行うかは、あなたが既に立てている仮説によります。あなたの仮説はなんでしょうか？\n\n　・生活不規則　　　→　　　便秘\n　・バイト過多　　　　→　　　腰痛\n　・飲酒　　　　　　　　→　　　頭痛\n\n3) 四分表の作成\nどれかの仮説に基づいて、４分表を作ります。\n\n　　　　　　　　　　　　腰痛（＋）　　腰痛（－）　    合計\nバイト過多（＋）　　　18　　　　    　22　　　　　　　40\nバイト過多（－）　　　12　　　　　    48　　　　　　 　60\n合計　　　　　　　. 　　30　　　　    　70　　　　 　　100\n\nさて、この４分表からどのような結論が出せるでしょうか。\n\n３ 横断的研究における四分表を考える\n\n1)  四分表とは何か？\n\n四分表とは２行×２列のクロス集計表のことです。\n四分表は横断的研究だけでなく、他の疫学研究においても、項目同士の相関関係や因果関係を数値化して検討する際に用いられます。\n\n2)  四分表の作り方\n\n疫学研究における四分表では、「仮説的原因・要因曝露の有無（＋、－）」を行に、「仮説的結果・疾病の有無（＋、－）」を列に示す設定が好んで用いられます。\n\n　　　　      　疾病(+) 　  疾病(-)        合計\n曝露(+)       　a                  b               a+b\n曝露(-)           c                  d               c+d\n合計             a+c              b+d           a+b+c+d\n\n3)  四分表による傾向の把握と限界\nさて上記の例のように四分表を作ると、例えば「バイト過多である方が、バイト過多でない場合に比較して、腰痛が多いかもしれない」などと、仮説についてコメントすることができます。\nではこのコメントから一歩進んで、「“バイト過多”が原因となって“腰痛”が起こった」などと両者の因果関係を結論できるでしょうか。結論できません。\nなぜなら横断的研究では、すべての要因を同時に調査しているため、たとえば「バイト過多」と「腰痛」のどちらが先に起こったかを区別できないからです。\n横断的研究（アンケート調査）では“時間的前／後”や“原因／結果”の区別が難しく、因果の逆転（原因と結果の逆転）も起こりえます。\n\n（守山正樹）","subitem_description_type":"Other"}]},"item_3_heading_17":{"attribute_name":"見出し","attribute_value_mlt":[{"subitem_heading_banner_headline":"疫学マイクロレクチャー ;
