The acting head nurses of three MLU that had been operated for at least three years were interviewed using semi-structured interviews. Data was qualitatively analyzed.\nIn the preparation stage, nurse managers acknowledged five challenges, “Staff involvement in the decision to introduce MLU”, “Preparation of a system to introduce MLU”, “Negotiation with the hospital administration”, ”Preparation of necessary equipment for the MLU” and “MLU that considers obstetric outpatient needs”. Nurse managers decided to introduce MLU and preparation started with staff motivation and negotiation with medical doctors and hospital administration.\nDuring the establishment stage, nurse managers perceived necessary to “Revise MLU standards”, “Prepare of a system to introduce MLU”, “Improve MLU staff self-efficacy” and “Improve midwives clinical judgment skills”. In order to ensure safety in the MLU, nurse managers revised standards and strengthened collaboration systems with medical doctors.\nSince establishment and through MLU operation, nurse managers were aware of two challenges, “Importance of midwives education to maintain and expand MLU” and “Revision of MLU target population standards”. Nurse managers intend to guarantee the continuity and expansion of MLU system by promoting the revision of operation standards and the improvement of midwifery practical skills.\nThe results suggested that through participatory leadership approaches, nurse managers motivated staff and revised standards based on specific situations.
