@article{oai:jrckicn.repo.nii.ac.jp:00000308, author = {MASUNARI, Naomi and 増成, 直美}, journal = {日本赤十字九州国際看護大学紀要, Bulletin of the Japanese Red Cross Kyushu International College of Nursing}, month = {Nov}, note = {http://ci.nii.ac.jp/naid/120005406357, application/pdf, 近年、抗がん剤の処方に関する事故が多発しており、抗がん剤の誤投与や過剰投与による患者の死亡事故が後を絶たない。これらの事例では、レジメンの記載の曖昧さ、処方医のレジメンの読み違え等による瑕疵ある処方せんの作成・交付に起因するものが多い。指導医の教育・指導・監督の程度により、間違った投薬が阻止されたり、途中で修正されたりということもあるが、他方で不幸な結果に至ることもある。そこで、本稿では、より安全な医療環境を獲得することを目指し、裁判所の判断や事故調査委員会の報告書を手がかりに、調剤過誤事例における医療従事者間の責任の分配を検討した。瑕疵ある処方せんは、処方医の重過失によるものが多かった。医薬品払い出しの段階では薬剤師のチェックにより、治療中は看護師の患者観察により、多くの場合、重篤な結果は回避できるものと思われた。, In recent years, accidents related to the prescription of anti-canner drugs are frequently seen; and patient deaths due to overdose or incorrect administration of anti-cancer drugs are numerous. In most cases, prescriptions are ambiguously written by the prescribing physicians or misread and wrongly prepared and delivered by pharmacists. Orientation and supervision provided by medical advisors may prevent cancer patients from harmful reactions to medicine caused by wrong use. Although there are several reported cases in which mistakes were noticed and amended; several other cases have led to unfortunate and even fatal incidents. Court decisions and reports of the respective incident investigation committees were revised to analyze the distribution of responsibility among health care workers in case of in dispensing errors. Defective prescriptions were mostly due to negligence of the prescribing physician, problems related to drug dispensing were due to insufficient check by pharmacists and drug administration problems caused by nurses were perceived by patients.
