A病院が地域連携医療に取り組んでいること、またその内容については認知度が高かったが、地域連携室の業務や自立支援会議、アルコール・薬物研修会の認知度は低かった。日常の勤務のなかで地域と連携した看護が行えていると答えた対象者はほぼ半数であった。患者や家族に対する社会資源の提供ができていない、あるいは必要と答えた対象者が多くみられた。2.これからの取り組みについて 地域連携を進めていくうえで看護師に必要なことや地域連携医療を充実させるためには、院内の連携強化が必要と答えた対象者が多く、看護師としての院外での活動が必要と答えた対象者は少なかった。地域連携は看護師が中心的役割を担う必要性があると答えた対象者の多くが、県や市町村との連絡調整、公開講座・出前講座など地域活動の取り組みが必要であると答えた。【考察】1.現在の取り組み状況について 認知度の低かった内容については、個々の知識不足に加え会議・研修会が開催されていない、または特定の者だけが参加する形であることが認知度を低くしているのではないかと考えられる。研修や学習会で個々の知識向上をはかるとともに他職種との連携を深め、院内連携の活性化に取り組むことが必要である。また、患者や家族への社会資源の提供が必要と感じているが行えていない現状が明らかとなり、看護師の社会福祉に関する知識の向上が求められる。2.これからの取り組みについて
